The book's tone and content effectively make the author's point that psychotherapy is a powerful instrument which requires self-discipline, knowledge, no mean intellectual effort, and considerable technical skill, in addition to the therapist's sympathetic and considerate attitude. Beginning therapists will feel constructively challenged by this message and by the author's criticism of experiential and humanistic approaches for their antitechnical and antitheoretical biases. The book's organization broadly follows the phases of therapy with systematic discussions of the data to be understood, transference, resistance, countertransference, the therapeutic alliance, and special problems. A concluding chapter discusses general principles of interpretation, working through, and timing. A valuable feature is the author's annotated list of suggested readings at the end of each chapter. The book's greatest strength is the author's concept of the role of interpretation in the therapeutic process. When the Past Distorts the Present

This book describes a few dos and don'ts of what to say to a person who keeps seeing the present as new versions of his past. Instead of saying something like, "You're really angry at your father, not me," it's more validating to describe what's happening in the present and then to link it to the past, "Each time I begin to say something to you, you wince as though you expect me to say something to hurt you just the way you always expected your father would."
If the person tends to avoid learning from feedback or is persistent in their "transference resistence" and even encourages it, it might be because that's what's most familiar to them in relationships, "The next interpretive step was to try to help [her] understand the motives for her clinging to this kind of interaction ... 'Sometimes you struggle with me because you feel that, otherwise, you will not continue to feel involved with me, which is how you have described feeling during periods when your mother seemed ... uninterested in you. ...'"
When a person is getting close to discovering a deeper insight about themselves, they may "act out" as a way of avoiding the insight. "Acting out should be understood and interpreted as a particular form of resistance in which the behaviour functions ... as a means of avoiding the direct introduction of certain material ... Acting out should be taken to signify the near readiness of the material to enter consciousness ... Some theorists speak of acting out within the transference. They are referring to certain transference manifestations as a form of acting out certain wishes and fantasies with the therapist instead of remembering them in relation to parental figures ..." pg 143
Some interpretations are about letting a person see the active role they play in their situation: "You feel you end up with such men by chance when, in fact, you choose them because they feel safe to you."
Regarding psychic masochism: "Now, when you arrange to be taken advantage of, you feel mother's approval as a presence within you which keeps you from feeling alone." "You feel the treatment is leading you in the direction of a more open search for fun and relaxation, which threatens you with feeling alone, without mother's approving internal presence. So you ... renew your suffering in order to get her back."
It seems that the root of most inner conflicts comes from the degree to which a person identifies with, and seeks to separate from, an internal bond to the conditional love of a parent within one's mind.
If a working relationship is there with a friend or therapist ("a therapeutic alliance") and the timing is right, interpretations can be curative.
The book also covers a few defense mechanisms (including the use of jargon as an intellectual defense against feelings), dreams, slips of the tongue, how to deal with counter-transference, negative therapeutic reactions and a few historical notes on the talking cure process.
Solid guidance and examples for both those in the helping profession and for those on a quest for self understanding.
